The injury/illness is reported to your company’s designated representative and the WCB.

A Worker's Report of Injury or Occupational Disease (C-060) must be completed and sent in within 2 years of the date of accident. See the following links for a copy of the Worker’s Report of Injury or Occupational Disease:

An Employer's Report of Injury or Occupational Disease (C-040) must be completed and sent in within 72 hours of receiving notice of the work related injury/illness.
