A health care provider (doctor, occupational health nurse, physiotherapist, chiropractor etc.) gives medical clearance for a worker to undertake modified duties and identifies his or her medical restrictions.

Sample forms: 8 (required where additional information is required to make a return to work plan)

Sample forms: 1a, 1b – should be sent with worker each time he/she attends health care provider, for an update in their abilities and limitations, for the purpose of gradually increasing the worker’s duties toward full duties.
